The doctor reviews the findings with you and creates a treatment plan together with you, never for you. No pressure, no judgment.
Paperwork
Patient forms
You can download our new patient health record ahead of your visit, fill it out at home, and bring it along. That is one less thing to do in the waiting room.
Prefer paper? We will have a copy ready for you at the front desk.
